Main duties

An exciting opportunity has arisen for a Children's Asthma Specialist Nurse post, working across primarily North Staffordshire/Stoke-on-Trent but will include some South Staffordshire clinics (half day a week).

The post holder will work as part of the Community Children’s Nursing Services providing expert nursing care and support in partnership with the child and family. The Specialist Asthma Nurse will provide specialist nursing care and support to children and Young People with a diagnosis of Asthma or for those meeting the criteria for likely asthma. This will include management of a caseload of targeted patients through the provision of Post discharge care, ongoing Community follow up and support, development of robust care plans that promote self-efficacy, liaison with the patients General Practitioner and other Health and Social Care professionals to ensure children and young people with Asthma achieve their maximum potential.

The Asthma Nurse Specialist will ensure compliance with Best Practice standards.

Job description and main responsibilities

Assess, plan, implement and evaluate programmes of care for children and young people with Asthma and their families, actively seeking evidence based practice to achieve both good clinical and individual developmental outcomes

  • Carry out skilled nursing care, efficiently utilising available resources and evaluating the effectiveness, providing clear and concise documentation.

  • This will include the reporting and recording of any changes in the child or their family and initiating action as required. Facilitate family centred care by providing accurate and appropriate information and support to families, involving them in all aspects of care.

  • With appropriate education and training, develop appropriate clinical skills for example in health promotion, teaching and assessing families, advanced respiratory assessment and spirometry.

  • Assess and advise in the management of children within their home or school environment, who are experiencing Asthma management problems.

  • Liaise and communicate with the multidisciplinary and interagency professionals across primary, secondary and tertiary care to ensure ongoing support and care of children.

  • The post holder will work primarily with the established Paediatric Respiratory service within Staffordshire Children's Hospital at Royal Stoke

  • Take responsibility for a defined caseload

  • Working autonomously and developing time management strategies to fulfil commitments to both clinical work and other responsibilities.

  • Able to respond to referrals within the service standards

  • Dealing with difficult decision-making including safeguarding and working across Trust and organisational boundaries.

  • Need to be flexible and responsive to patient requirements, necessitating frequent reorganisation of working schedule at short notice on a daily basis.

£33,706-£40,588

Hours

Full-time 37.5 hours per week. Permanent.
