Job overview

An exciting opportunity has arisen to join an innovative and dynamic team within the Frailty Virtual Ward in the community. We are looking for an experienced and motivated Band 5 Registered Nurse who is passionate about service development and providing high standard of patient centred care. You will be working closely with a friendly team of Multidisciplinary staff including Doctors, Nurses, Occupational therapists, Physiotherapists and HCA's / rehabilitation assistants who are committed to maintaining independence, promoting self-care and assisting patients to achieve their goals. The successful candidate will be hardworking, reliable and able to use their initiative and work flexibly to respond to the service needs.

The Frailty Virtual Ward covers the hours 8.00 - 20.00 seven days a week 365 days a year. We are committed to upskilling and supporting peoples professional development.

Main duties

As a member of our multi-disciplinary team, you will work to meet the needs of a complex community, delivering patient care supporting early discharge and avoiding admissions to the acute sector.

Main duties include:

  • End of life care

  • Managing the deteriorating patient,

  • Complex and simple wound care,

  • Observations and ECG's

  • Venepuncture,

  • Utilising advanced physical assessment skills

  • Pressure area care

  • Falls assessments

  • Taking Medical and Social History

  • Completing Comprehensive Geriatric Assessments

  • Attending MDT's

  • Liaising with other community teams and acute trusts

  • Line management

  • Health promotion and education

£27,055-£32,934
