Gloucester Integrated Community Team- Community Nurse Lead.

We are two Community Nurse Leads working in the Gloucester Integrated Community Team looking to welcome a third member to join us in leading & delivering a wide range of community  nursing care in this diverse, mainly urban  environment. We are very proud to work as part of a committed and knowledgeable multi-disciplinary team with an amazing group of dedicated staff across our nursing, Occupational Therapy, Physiotherapy, Reablement and admin teams. Main duties of the job

You will clinically lead, supervise and manage a community nursing team working closely alongside them each day providing both direct and indirect patient care.

Apply your Clinical operational experience, maintaining and improving quality, monitoring standards utilising  data, audits and reports.

Identify service delivery development or system opportunities working with our partners across Health & Social care and community networks.

Build Key relationships with internal and external stakeholders to promote and build community nursing services .

You will join an established highly experienced peer group of professionals leading Community Nursing, Physiotherapy, Occupational Therapy and Home first/Reablement service. All  are committed to maintaining and enhancing a community based multi-disciplinary offer for the residents of Gloucester with an embedded Health Coaching, personalised, strengths based approach with the person in their own home and community at the centre.

Qualifications

Essential criteria

Specialist Practitioner Qualification or 3(50%) of required degree modules completed in the past 2 years

NMC registration

Desirable criteria

Hold a recognised teaching and assessment/ mentoring qualification

Experience

Essential criteria

Post registration Community Nursing Experience

Clinical team leadership including professional and clinical supervision to staff at a variety of level

Desirable criteria

Working as part of a multidisciplinary team

£41,659-£47,672

Hours

Full time - 37.5 hours per week (Monday to Friday with 1 weekend in 15 on call)
