Job summary

Older Persons Mental Health Team in Portsmouth are looking for a motivated, enthusiastic

practitioner to join our friendly team, who provide skilled assessments and treatment interventions

to the older adult.

To be a successful applicant you will need to be a registered nurse, preferably RMN, with

experience in the care of older adults with mental health problems. You must be enthusiastic in

working with older adults to provide compassionate care to patients and their families,

and have knowledge of organic and functional illness with a good understanding of physical health

and frailty.

You will be required to work within the wider MDT team, implementing and evaluating care,

managing your caseload, supervising HCSWs, junior staff and students within the team.

You will play an important role in our team, we aspire to provide excellent care to our patients and

strive to improve and develop our service in line with national and local guidance.

Main duties of the job

*To meet the needs of patients and their families to achieve improved health outcomes and promote health and wellbeing while demonstrating care, compassion, competence, communication, courage, and commitment. Care is based on comprehensive assessment, ensures continuity and is patient/client centred, culturally sensitive and evidence-based and which promotes independence.

*Responsible for co-ordinating the work of the team, influencing clinical decisions and monitoring the quality of patient care provided through utilising and maintaining expert clinical knowledge relevant to own field of practice, underpinned by theory and expertise.

*Manage a caseload/named nurse.

*Supervise junior members of staff, students on placements.

*To develop and maintain a culture of service improvement which promotes quality and safety and striving to do better.

*This is in line with the requirements set out in the NMC code which require all registered nurses to: Prioritise people; Practice effectively; Preserve safety; Promote professionalism and trust.
