We have two exciting opportunities for highly skilled and enthusiastic Band 6 District Nurses to join our Warwick and Kenilworth team based at Acorn House in Warwick and our Leamington team based at Whitnash Lodge.

You will contribute to the future workforce planning and pathway development of Routine Care and work closely alongside other services internally within our Urgent Community Response Team, Specialist Nurses, our Acute Trust at Warwick Hospital, and externally with the Local Authority, Hospice partners and GPs.
You will be supported in an environment where training and development opportunities are actively encouraged. With internal courses available in Leadership and Management and clinical simulation training as well as external courses supported by Coventry and Oxford Brookes University.

Job duties
The District Nurse is responsible for the assessment and management of the health needs of an identified population and for the development, implementation and evaluation of programmes of care within a range of target groups including end of life care, complex health needs and patients identified as being at risk of hospital admission.
The District Nurse will clinically supervise and provide support to a team off staff - being responsible for the development and competencies of staff within their team. To include performance development reviews, clinical supervision and recruitment and selection.
