Job summary

As Homefirst CERT teams, we provide a short term assessment of needs and provision of care whilst longer term arrangements are established.

We focus on facilitating discharges by providing targeted rehabilitation/assessment of health needs to support patients to return home from hospital in promoting independent living also preventing unnecessary admissions by supporting those that require a fuller assessment of health needs or care.

We are looking for flexible and organised Band 5 Nurses to join our Community based team based in Stratford.

Ideally you should have related experience, preferably in the community and have some knowledge and skills in managing the care of long term conditions and end of life patients.

Working in the community means you need to be flexible and able to travel, a clean driving licence is essential.

Main duties of the job

The community staff nurse is a member of the Integrated Health Team, whoundertakes professional duties in the client's homes and clinics to ensure provision of high standard nursing care and health promotion. To work across professional boundaries, providing comprehensive health care that is sensitive to the needs of the local population.
