Job summary
To provide specialist assessment to people of all age groups with the service who may, because of complex physical and mental health needs, be at risk of losing their independence.
To provide advice and support to all staff in the multi-disciplinary teams and to GPs and medical staff.
To identify patients that are referred to the service who are cognitively impaired but do not have a mental health diagnosis and refer to GPs for early diagnosis.
To identify training needs and set up training to meet those needs. To link with the Sussex Partnership Trust (SPT) to support the development of care pathways between the services. SPT will provide clinical supervision working in partnership with management in community nursing.
Main duties of the job
To receive and respond to referrals from the community and community beds.
To be responsible for assessing mental health needs.
To undertake risk assessments and devise risk management plans to minimise identified risks
To actively contribute and support care plans to meet mental health needs
To identify services to support the individual on a long-term basis and actively ensure a seamless transfer of care to mental health services
To liaise with social workers and other professionals to plan any longer term needs on discharge from the service
To provide advice to GPs on management, treatment and medication
To be part of the multi-disciplinary team and liaise with all professionals across the service
