Job summary

We are pleased to share the opportunity for an experienced Mental Health Nurse to join a well established Community Mental Health Team. The successful candidate would be joining a fantastic team who support each other and strive to deliver an exceptional service for its patients. We support the development of Clinical Skills so that we can deliver evidence based interventions for patients including accessing training in Structured Clinical Management, EMDR Therapy, DBT Skills, Solution Focused Therapy, Brief Cognitive Behavioural Therapy and Complex Risk Management. The successful candidate would therefore be joining a highly skilled & supportive team with a range of disciplines and expertise available within the team including nurses of varying backgrounds, occupational therapists, clinical support workers, associate practitioners, psychotherapists, psychologists, psychiatrists and administrative staff.

We have robust systems in place to ensure that care can be delivered safely for patients and staff & we currently are managing full time care coordinator caseloads at a capacity of approximately 20 to ensure you have the opportunity to have an impact on a service users recovery journey whilst being supported to develop yourself. In addition we are looking for a full time nurse to join our Standard Care nurses supporting patients who no longer require enhanced care such as through Clinics, Short Term Interventions and Transition work back in to Primary Care.

Main duties of the job

There are two functions to the Community Mental Health Nurses role. You may be required to fulfill the care coordinator role for patients receiving Enhanced Care. This involves working alongside the patient's consultant psychiatrist to coordinate a person centered and collaborative care plan. You would be responsible for coordinating their care and treatment in line with evidence based practice and appropriate guidelines. All patients under Enhanced Care require a care plan and risk assessment which as care coordinator you will have responsibility for. Or you may support patients on Standard Care by facilitating clinics (Depot, Clozapine, Assessment), perform short term pieces of work with patients to support discharge or manage crisis presentations. This also includes triaging incoming requests to the team such as prescription queries, multiagency meeting requests etc. To support you in this role we have developed a Triage system that helps you identify appropriate timeframes for responses so that you can prioritize clinical duties in an evidence based way.

You will form part of an MDT and provide consultation and support to the team during MDT forums such as a weekly case discussion meeting & daily Safety Huddle. Often from these meetings come ad hoc clinical duties such as liaising with other services, supporting a colleague with a double up visit or implementing risk management plans.
