Job summary

We are looking for an enthusiastic, honest, self-motivated colleague who is able to provide high quality person centred care to patients, their families and carers within the Specialist Dementia and Frailty Service Multi-Disciplinary Team (MDT) in a range of settings in the West Essex Area.

Main duties of the job

The SDFS team provide a comprehensive assessment, diagnostic and treatment pathway supporting patients of any age with dementia as well as older adults over 75 years of age, who have functional mental health needs with an associated age related frailty.

We work closely with our EPUT community specialist practitioners and Essex County Council colleagues, local organizations as well as the voluntary sector to support the holistic needs of the patient with a person centred collaborative approach.

The successful candidate will be trustworthy, positive about change and will be passionate about the services we provide. The successful candidate will be able to work independently and autonomously, as well as part of a team therefore will need excellent verbal and written communication skills and will have excellent interpersonal skills and a zest for delivering the highest level of patient care and service delivery improvements.The successful candidate will be able to adhere to the trust core values of being compassionate, open and empowering.
