Successful candidates will support patients in their own homes, care homes, the travelling community and community clinics. Working Pattern:
The service operates between 08:00 and 20:00 hours, seven days a week, and you will be required to work shifts between these times, but we are willing to explore candidates who would like to consider flexible working, part time, full time and possible bank hours.
Main duties of the job
Assess, prescribe and delivery care for patients residing in the community setting to support self-management and improve quality of life.
Provide education, support and prescribing planned care for individuals and their carers/family with long term conditions, such as diabetes and wound management.
Support patients at the most sensitive periods of their life, including palliative care and syringe driver management.
Work as part of a multidisciplinary team, linking with the local Primary Care Network, wider community stakeholders and your immediate team which consists of Community Nurses, Community Matrons, Community Nurses, Physiotherapists, Occupational Therapists, Assistant Practitioners and Healthcare Assistants.
You will be supported to maintain competencies and training in specialist skills, such as bowel care, leg ulcer management, catheters, venepuncture, IM injections etc.
