We are looking to recruit enthusiastic, experienced and motivated Community Nurses to join our community team. The successful applicants will be joining an innovative and forward thinking team, contributing to the delivery of high standards of clinical care across the community of Sheffield.
Main duties of the job
These posts will deliver palliative and end of life care as part of the St Lukes multidisciplinary community services delivering specialist skills in patient centred holistic assessment, symptom management, and effective clinical and advanced care planning. The role also includes participating in education and training both internally and externally to the organisation. The post holder will work under the direct clinical leadership of advanced practitioner level community nurses and will be responsible to the Matron Community Team and Active Intervention Centre.
Applicants for these posts will be first level registered nurses with significant post-registration experience with a minimum of one years palliative care experience. You will be motivated, committed and have a flexible approach with a passion to progress. Evidence of continuing professional and personal development at diploma level is essential and you must be a car driver and owner in order to be considered.
All posts within the St Lukes are eligible to join the St Lukes Pension Scheme. Please note that St Lukes is not part of Sheffield Teaching Hospitals or an NHS employer and the terms and conditions differ to the NHS benefits package.
