The Directorate is pleased to offer an exciting opportunity for an enthusiastic Band 6 Nurse to work as a member of the Urgent Community Response team to support the Coventry Community Out of Hospital offering and wider admission avoidance strategies.
The successful applicant will be working as a Band 6 Sister/Charge Nurse supporting the Urgent Community Response pathways to reduce unnecessary hospital admissions and contribute to effective admission avoidance. The role incorporates the use of high-level holistic assessment with the use/willingness to work towards the health assessment qualification to support patients presenting with a wide variety of acute medical conditions within the community setting.
You will be responsible for facilitating effective teamwork and provide mentorship, teaching and support across the multidisciplinary team. You will be working alongside a highly skilled workforce including Advanced and Clinical Practitioners, Therapists and liaising closely with other members of the MDT including District Nurses, GPs, and Care Navigators

Main duties of the job
As a valued team member your development will be supported through a robust induction programme, a value-based appraisal, one to one meeting, regular in-service training and clinical and managerial supervision. You will be work out of a multi professional base so there is opportunity for joint working and collaborative learning across a range of Health Professionals who are supporting and delivering high standards of clinical care. Training opportunities are supported by our Practice Based Facilitators and Talent for Care team and is actively encouraged by Team Leads.
Our working hours are over a 7 day flexible service, covering shifts from 8am-8pm. Applications for flexible hours will be considered on an individual basis. Car owner/driver is essential.
