Come and join a forward thinking dynamic Older Persons Community Mental Health Team who provide person centred and tailored care to each individual patient.

The Older Persons Community Mental Health team, based at Havant Health Centre are looking for a Senior Community Mental Health Nurse/Practitioner, who will work as a core member of the Community Mental Health team to co-ordinate the care of complex patients, working alongside other health and social care professionals.

The Team covers a wide geographical area including Havant, Hayling Island, Emsworth and Waterlooville.

Working alongside the Team Manager and area Consultant Psychiatrist, you will formulate the care, risk management and crisis plans in accordance with the identified need of the patient. This will be holistic based, person centred and recovery focussed in collaboration with the Patient and where appropriate their relative and carer.

You must be a Registered Mental Health Nurse or Social Worker, with post registration experience of working with Older Adults, Adults or working in the wider Community Mental Health services.

Main duties of the job

The Senior Community Mental Health Practitioner will work as a core member of the Community Mental Health team to co-ordinate the care of a complex caseload of service users supervising a Band 5 Community Practitioner/Band 3 Community HCSW where appropriate.

Working under the supervision of the Deputy Team Manager/Team Manager the practitioner will formulate care and risk management plans in accordance with identified need and ensure that these are recovery focussed and developed in collaboration with the service user and where appropriate their relative and carer.
