Job summary
An exciting opportunity has arisen for a Band 5 Registered Nurse to join the @Home Service for a 12 month secondment or 12 fixed term position.

The post holder will work within the @Home Service, which provides a comprehensive multi-disciplinary assessment service for adults presenting with complex needs in the community who are likely to be admitted to hospital in the absence of a timely health intervention.
We also promote earlier discharges from District General and Community hospitals, through this revised approach to earlier intervention, joint working and flexible deployment of resources patients are able to return to their own environment earlier with support from the service rather than face a prolonged admission.
The post holder will work closely with multidisciplinary team colleagues, the service operates a 7 day nursing response to cover a range of interventions and assessments which enables patients to be supported within their own environment.

Main duties of the job
The post holder works within the @Home Service, which provides a multi-disciplinary assessment and timely response service (within 4hrs) for those individuals who are likely to be admitted to hospital in the absence of timely health intervention. The service operates across the Rhondda/Taff Ely Locality and the Merthyr/Cynon Locality 7 day a week.
The post holder will work closely with medical and therapy colleagues in the provision of a comprehensive multi-disciplinary assessment service for adults presenting with complex needs in the community or secondary care setting.
Take delegated responsibility from the Team Leaders/Advanced Nurse Practitioners for managing patients, within a variety of settings, and will be responsible for providing timely clinical assessments and treatment plans for these patients. For additional information please see the attached JD.
