Our Community Nurse Teams are responsible for the assessment of nursing care needs, implementation and evaluation of care programmes and the delivery of high quality holistic care within the community setting, and within our catheter and leg ulcer clinic's. You will be expected to support your peers, the team leader and the organisation in promoting the Trusts values and behaviours.

The hours of work are between the hours of 08:00 and 17:00, over 7 days.

You will need to be able to work independently, possess good time management and organizational skills and be expected to be innovative and flexible to work in other areas if required to meet service need.

Please note whilst main base will be in Northwich you may be required to work across the footprint of Central Cheshire Integrated Care Partnership.

Main duties of the job

We are looking for Band 5 staff nurses to support the Caseload holders in delivering high quality, efficient and safe care. We are looking for Nurses who are - newly qualified, community experienced or have no previous experience, for our team it is about the individual.

We are looking for an enthusiastic, person who possesses both clinical and digital skills to support in the promotion of quality improvement for a local population. Main duties will involve wound management, chronic condition management, catheter management and palliative support and end of life care. Our patient groups are varied and often complex.
