Job summary

Following recent investment in the local Tissue Viability Service within Somerset, an exciting opportunity has arisen for a Band 5 Tissue Viability Support Nurse, within the community team.

This post is to cover the Central and East Somerset area.

Somerset NHS Foundation Trust is formed from recent mergers of Musgrove Park Hospital, Yeovil District Hospital and Somerset Partnership (Community Trust); this is therefore a great time to join the specialism, as we look to transform the tissue viability provision across the healthcare system within Somerset.

The successful candidate would ideally have experience of elements of the healthcare environments within Somerset and the digital systems used.

As an independent worker, excellent communication and organisational skills are essential to achieving the high standard of evidence-based care expected by the Trust.

This is a 37.5 hours per week role; hours and base can be negotiable for the right candidate.

Main duties of the job

Under the direction of the senior members of the Tissue Viability Team to be responsible for the assessment of both complex and simple care needs relating to Tissue Viability.

To support members of the Multi-disciplinary Team with the management of their patient's tissue viability, including pressure ulcer prevention/management care needs through support visits, advice and education.

Contribute to the Trust governance process for the management of pressure ulcer incidence by undertaking the validation process on the Trust clinical incident system.

To support the Tissue Viability Education programme for the Trust.

To support the Tissue Viability Specialist Nurses with the ongoing monitoring of patient's with tissue viability care needs.

To support the development of the use of continuous pressure monitoring element of the service.
