We are looking for enthusiastic and motivated band 5 Registered Nurses who are committed to delivering excellent patient care to join our Integrated Network Team's (INT) across Bristol & East Bristol.

We are currently recruiting in the following areas:

  • Central Bristol - Covering the inner city, Eastville, etc

  • St George - Covering St George, Hanham, Barton Hill, etc

  • Fishponds - Covering Fishponds, Mangotsfield, etc

We provide an urgent and planned nursing and therapy service to these local areas, providing high levels of care to our service users in their own homes.

We welcome applications from staff currently working in an urgent environment who possess the necessary skills to prevent hospital admissions. However, full training and development will be provided in all areas of extended practice that a community role requires.

We deliver our interventions across several pathways; urgent and planned nursing and therapy, planned nursing and planned therapy, discharge to assess and prevention of hospital admission. You would have the opportunity to be involved in all pathways that we deliver.

Our service operates 7 days a week, between the hours of 8am and 8pm for planned nursing interventions and urgent responses, including interventions that prevent hospital admissions

Weekend working and late shifts (until 8pm) is sometimes required, however most of your working hours would be between 8am and 4.30 pm.

Main duties of the job

You will be visiting patients in their own home or residential setting, providing skilled nursing care, supported by senior nurses in the team. Your key responsibilities will include:

  • Providing skilled nursing care to patients in their own homes and some clinics and care home settings.

  • You will undertake nursing roles such as administration of medication, pressure ulcer prevention and management, wound care, compression bandaging, catheterisation, facilitating early hospital discharge and preventing hospital admissions.

  • Liaison with the multidisciplinary team is a key part of your role.

  • The team works collaboratively across BNSSG, primary care, social care, the third sector and most importantly with patients and carers to provide the best possible care and treatment to ensure people can remain as independent as possible.

You will have the opportunity to work autonomously and collaboratively within the Integrated Network Team which comprises of Advanced Clinical Practitioners, Community Nurse Team Leaders, Senior Therapists, Community Nurses, Assistant Practitioners, Nurse Associates, Healthcare Assistants and Rehab Support Workers.
