We’re looking for Community Nurses tojoin our North Kent Community Nursing Service, Out of Hours Service, with a base at Livingstone Community Hospitalto help us continue to change things for the better. The community nursing team works across Dartford, Gravesend and outlying villages between 20.00-08.00.

Main duties of the job

As a Band 6 Out of Hours Nurse you will

• Deliver nursing care to housebound adults within the community

• Managing small team of nurses

• Be responsible for contributing to the holistic assessment, planning delivery and evaluation of care to patients within their own homes.

• Implement, monitor and maintain high standards of care delivery at all times.

• Care for patientsand their families in the way thatyou believe is best

• Deliver our high service standards, sharing bestpractice, and actively improving the way we work.
