Forest Neighbourhood Team is a friendly, supportive and hardworking team who are looking for enthusiastic individuals to join us. The team operates from 7:30am 6:30pm, Monday to Sunday including bank holidays). Shift patterns include 07:30-15:30, 08:30-16:30 and 10:30-18:30 on a rota basis. Flexible working opportunities such as compressed hours are available.

We are looking to recruit a senior community nurse on a full-time or part time basis, who is innovative and keen to join the team in supporting us to deliver high quality care, and to continue our development of integrated care in the community. The role is varied and rewarding.

We are looking for an experienced passionate nurse who is excited by change and is keen to help shape a new way of delivering nursing care in the community. We are a team of health care professionals who are focused on compassionate care. We work closely with our GPs and work hard at developing relationships with other primary care services, drawing on the skills of our MDT colleagues to manage complex patients and ensure our services are responsive and patient centred.

Main duties of the job

Your typical working day will include home visits for adults over 18 years needing a range of nursing interventions. These include caring for people reaching the end of their life, managing their pain relief and other symptoms, all types of wound care, IV therapy and other injectable drug administration and holistic assessment of a persons needs, taking in to account their environment and social circumstances.

As part of the role, you will support students (both pre and post registration) in a supervisor and / or assessor capacity. You will also be required to support band 5s and HCAs.

The senior nurses take responsibility for clinical leadership within the neighbourhood team for specific areas of interest such as leg ulcer management, palliative care and virtual ward. You will oversee a caseload of patients and assist in decision making regarding their care. You will attend monthly MDT meetings within the wider PCN and regular palliative care meetings with a specific GP surgery.

You will be required to undertake the Liaison role on a weekend and bank holiday on a rota basis and regularly undertake the triage co-ordinator role.
