An exciting opportunity has arisen for a Band 5 Community Registered Nurse for the District Nurse (DN) Treatment Clinic / Lower Limb Service (LLS) working across Livewell Southwest. The LLS is a National Wound Care Strategy Programme (NWCSP) Implementation Site 1st tranche for the NWCSP Lower Limb Recommendations, uses digital wound imaging, actively participates in research, & has also recently been shortlisted for a HSJ award.
The LLS offers a specialist service and the successful applicant will need to demonstrate a strong interest/knowledge in Tissue Viability & lower limb management and all aspects of lower limb health & skin care. Working clinically under the guidance of Community Sisters & Team Managers. Effective communication & teamwork are key. Health promotion/patient education is an essential part of this role, as is collaboration with other allied healthcare professionals. We work closely with Vascular, Podiatry & Marjon University's Sports Therapists deliver our Wellbeing programme.
The DN Treatment Clinic provides care to ambulatory patients that is unavailable at their GP surgeries: pleural drain management, urinary catheter management, routine tracheostomy care, nephrostomy care & Topical Negative Pressure Therapy wound dressings.
A full driving licence and access to a car is essential to ensure that the post holder can travel to clinics within the Livewell South West Footprint.
Main duties of the job
This post holder will work as a member of the DN Treatment Clinics/Wellbeing Lower Limb Service and will need to demonstrate a history of interest in lower limb management and Tissue Viability.
The post holder is supported by the Community Team Manager and Specialist Service Lead and is responsible for providing specialist advice and information for the holistic assessment and management of Lower Limb disease. They will need to be highly skilled in compression bandage application techniques and Doppler assessment, measurement and interpretation of the ABPI. Underpinning aetiology and management of patients with leg ulcers. The post holder will be required to make contemporaneous recommendations for the management of lower limb conditions and implementation of an appropriate care plans. Demonstrating the principles of wound healing and management, the aim of which is to enable patients to enjoy maximum independence and optimum quality of life.
They may be required to practice independently during their working day as well as within a team.

Daily duties involves prolonged bandaging & the work is physically demanding. Work takes place in a variety of treatment clinic settings across the Livewell footprint, including an informal setting within a village hall.
This role may not be eligible for sponsorship under the Skilled Worker route.

Dimensions & Context of Role:
To improve leg ulcer healing rates across the Livewell Southwest footprint as agreed by the CCG.
To provide leadership and management within the team.
To promote the ongoing development of the Lower Limb Service.
To oversee and delegate responsibilities responding to service needs as required.
To proactively seek feedback from service users to enable evaluation of the service.
The post holder will have a clinical role within the treatment clinics and will also have the opportunity to participate in the delivery of educational training packages, evaluations and clinical trials.
Responsible for specialist assessments, planning care and making recommendations for complex leg ulcer management and tissue viability needs.
To work alongside the voluntary sector, 3rd party and primary care. Providing appropriate specialist skills to ensure care packages are responsive to individual needs of patients within designated Wellbeing Lower Limb Clinics.
To work across the DN Treatment Clinics. Focusing on Lower Limb Care and sharing best practice.
