You will work as part of an integrated health and social care team providing nursing assessment, care and treatment to adults and older people living at home who have physical health needs which may be complex and long term.
Shifts are worked on a 5 day rota'd basis, between Monday-Sunday with hours of 08:30-17:00. Weekend working is also required on a rostered basis. Experience within the community would be desirable however, everyone has to start somewhere and we want to help you grow and develop as a Community Nurse.
We need you to travel across the city to visit our patients so ideally, you will have access to your own car. We also have pool cars which can be used by people with full UK driving licences and if you want to use your own, you can claim mileage!
What we really need from you is the drive to deliver the best quality and patient centred care.
Main duties of the job
The post holder will take responsibility for managing, co-ordinating and delegating nursing and care interventions within an integrated health and social care team and managing the transfer of care into the Locality Health Teams and District Nursing services.
You will participate in the development, implementation and evaluation of the delivery of health care services and be part of the development of health services and inter-professional working.
In this role, you will have responsibility for providing the highest standards of patient care through on-going assessment, planning, implementation, and evaluation of programmes of care.
Working with the Locality Health Team and other agencies you will use your advanced person-centred assessment skills for the health and well-being of patients with complex needs.
To provide a nursing service in the patient's own home and other community settings to include:
*First contact: acute assessment, nursing diagnosis and care, treatment, and referral.
*Palliative Care: symptom management including assessment of acute symptom onset, care for the patient and their family/carers.
*Continuing Care: Intermediate Care, rehabilitation, chronic disease management and contributes to the delivery of the National Service Frameworks.
