We have an exciting opportunity for Band 6 Registered Nurse to join our dynamic, forward thinking Community Nursing Team.

Our aim is to provide a high standard of evidence based holistic care to housebound patients within their own homes so as to avoid unnecessary hospital admissions.

The successful candidate will have a general knowledge of current best practice in Community Nursing and hold a District Nurse degree or equivalent. They will also be an effective leader with excellent communication skills and have the ability to manage a caseload of patients.

We will provide you with a structured induction and training period to ensure you have the opportunity to develop skills and knowledge. We encourage staff and support continuing professional development progression.

The service operates seven days a week - shift patterns are normally 8-4 & 10-6 with 5 shifts per week. Flexible working requests would be considered for suitable candidates.

The requirements are that you should hold a valid UK driving license and be willing to travel across Stratford and surrounding villages. A mileage allowance is available.

Main duties of the job

To be responsible for the assessment and management of the health needs of an identified population and for the development, implementation and evaluation of programmes of care within a range of target groups including end of life care and patients with long term conditions and complex nursing.

To prescribe using the Nurse formulary and ensure that nurse prescribing is undertaken in a safe, cost effective manner.

To provide managerial support to the team including Performance Development Reviews, Clinical Supervision and Recruitment and Selection.

To provide leadership to the clinical team.
