Your role will require close collaboration with GP's, health and social care colleagues including therapists and voluntary organisations. We would like you to possess commitment to team working and embrace innovative delivery of excellent standards of care. You will need to have a real enthusiasm for community nursing as an autonomous practitioner and a desire for ongoing professional development.
