UCR is a service that is dedicated to providing care at home to patients who require this rapid assessment. UCR are dedicated to supporting patients where they want to be. We work alongside the MDT to prevent admissions to hospital to support patients to stay in their own homes.
The Urgent Community Response Team are looking for Community Staff Nurses to join our Team in Wakefield and our surrounding areas. Working as part of our team, you will have the opportunities to: develop or extend your clinical knowledge and experience by working across a range of clinical settings; develop extensive clinical assessment such as hospital avoidance, managing long term conditions, wound management, and end of life care; and to plan and deliver complex care plans.
Main duties of the job
The successful applicant will have good communication skills, IT skills, and a flexible attitude to working patterns and environments. Your work pattern will include shifts and weekends. You should also have a full clean driving license, and access to a vehicle for work purposes.
Experience of working in the community is advantageous but not essential as a full induction program will be offered.
