Community Nursing - bringing the hospital to our patient's home.
Solent NHS Trust is looking for inspirational registered nurses to deliver a high standard of quality care to patients within the community setting. Community Nurses work autonomously and as part of a team and care is delivered in the patient's own home environment (including residential care) and designated treatment rooms. Our nurses are responsible for a manageable caseload, working across the city ensuring our patients are receiving the best quality care from the comfort of their own homes and thereby avoiding hospital admission.
You will work within a team that is based at either Bitterne Health Centre, The Royal South Hants Hospital or Broadcast House (by The Western Community Hospital) all located in Southampton. Our working patterns are across a 7 day week. Our core shifts are 0830 - 1630 and 1400 - 2200 hours, but we are open to discussing this. There is also a requirement that you will work 1 in 4 weekends as well as bank holidays. Your job has to work around you too so let's talk about how we can get this right for you and our patients.
Main duties of the job
Solent's Community Nursing Service work with adults who need support for a multitude of health conditions. These include:
*Long term conditions management
*Acute wounds and complex wound care
*Palliative and end of life care, including bereavement support.
*Facilitating discharge from hospital, including drain management, administration of prophylactic medication
*Continence care, including all types of catheter care and bowel care
*PICC line management
*Administration of a variety of medications via different routes
Although a lone worker, you will be supported by an experienced and nurturing team who are always available, to allow you to develop your existing skills into becoming an autonomous practitioner in the community. You will be provided the training you require to maintain and develop your clinical skills. This will be achieved by attending appropriate training and working with our Educators in Practice and other staff within the team.
Solent's Community Nursing is much wider than the nursing team. We work alongside our colleagues within Social Care, our Internal Safeguarding Team, specialist services (both within Solent and external providers) to ensure our patients are provided with the care they require. Working collaboratively enhances staff development but also ensures that our patients are at the heart of everything we do.
